Our outreach workers provide one-on-one support with housing, income, community resources, and more by assessing each client’s specific needs and challenges to develop individualized care plans based on available services and the client’s short and long-term goals.

Case management-focused approach
Centered around client needs, our individualized approach enhances our Outreach team’s ability to provide ongoing support and ensure clients maintain stable housing. Our support continues even after clients secure housing and once immediate physical and safety needs are addressed.
Assistance with essential life skills
Our Outreach team assists clients with skills such as budgeting, grocery shopping, and home maintenance, ensuring they have the necessary tools to thrive in their new homes.
